UAT TEST VERSION — DEVELOPMENT ONLY 1. I certify that the information and supporting documents I provide in this membership application are true, complete, and correct to the best of my knowledge. 2. I authorize QPGOE MPC to verify my identity, employment or eligibility information, references, and documents submitted in connection with my Associate Membership application. 3. I agree to comply with the applicable by-laws, policies, rules, membership requirements, contribution requirements, and lawful decisions of QPGOE MPC that apply to Associate Members. 4. I understand that submission of this application does not automatically guarantee approval of my membership. Final acceptance remains subject to review and approval by the authorized officers of QPGOE MPC. 5. I confirm that typing my complete legal name in the Electronic Signature field constitutes my electronic signature for this membership application and related membership acknowledgments. 6. I consent to the collection, use, verification, retention, and processing of the information contained in my application for membership evaluation, cooperative administration, regulatory compliance, audit, security, and official recordkeeping. 7. I understand that the Terms and Conditions applicable to Associate Membership are maintained separately from the QPGOE MPC Share Subscription Agreement. Acceptance of these Terms and Conditions does not replace the separate acceptance of the applicable Share Subscription Agreement. 8. I acknowledge that this version is being used only for Development/UAT testing and must not be treated as final Board-approved production content.

Pinatutunayan ko na ang lahat ng impormasyon at mga dokumentong aking isinumite sa aplikasyon para sa pagiging kasapi ay totoo, kumpleto, at tama ayon sa aking kaalaman. 2. Pinahihintulutan ko ang QPGOE MPC na beripikahin ang aking pagkakakilanlan, impormasyon tungkol sa trabaho o kwalipikasyon, mga sanggunian, at mga dokumentong isinumite kaugnay ng aking aplikasyon bilang Associate Member. 3. Sumasang-ayon akong sumunod sa mga naaangkop na by-laws, patakaran, tuntunin, kinakailangan sa pagiging kasapi, kinakailangang kontribusyon, at mga legal na pasya ng QPGOE MPC na naaangkop sa Associate Members. 4. Nauunawaan ko na ang pagsusumite ng aplikasyong ito ay hindi awtomatikong garantiya ng pag-apruba sa pagiging kasapi. Ang panghuling pagtanggap ay nakasalalay pa rin sa pagsusuri at pag-apruba ng mga awtorisadong opisyal ng QPGOE MPC. 5. Pinatutunayan ko na ang pag-type ng aking buong legal na pangalan sa Electronic Signature field ay magsisilbing aking elektronikong lagda para sa aplikasyong ito at sa kaugnay na mga pagkilala at pahintulot. 6. Pumapayag ako sa pangongolekta, paggamit, beripikasyon, pag-iingat, at pagproseso ng impormasyong nakapaloob sa aking aplikasyon para sa pagsusuri ng pagiging kasapi, pangangasiwa ng kooperatiba, pagsunod sa regulasyon, audit, seguridad, at opisyal na pagtatala. 7. Nauunawaan ko na ang Terms and Conditions para sa Associate Membership ay hiwalay na pinamamahalaan mula sa QPGOE MPC Share Subscription Agreement. Ang pagtanggap sa Terms and Conditions na ito ay hindi kapalit ng hiwalay na pagtanggap sa naaangkop na Share Subscription Agreement. 8. Kinikilala ko na ang bersyong ito ay ginagamit lamang para sa Development/UAT testing at hindi dapat ituring bilang pinal o Board-approved na production content.
